Do I need to register?

There is no need to pre-register. If it is your first appointment, please aim to be here 5-10 minutes before your appointment time as we have a short registration form that will need to be completed by you before your appointment or can be downloaded from our website.

If you chose us as your new GP, we can send a letter to your previous GP with your written consent to request that your medical records be sent to us.

If you have a medical card with a GP other than ourselves and wish to move to us, we can also send a form to the HSE with your request to for a transfer.
